The Omega Life Membership Foundation, Inc. (Foundation), which was established 39 years ago, has faithfully been fully engaged in and committed to philanthropy throughout all of its years of existence. It remains the intention of the OLMF to continue on this path and be a force for good and growth in the years to come. The IMPACT FUND is the first initiative promulgated by the Foundation’s Board of Directors. It has been designed as a “Donor Advised Fund”, which means that the designee of the grant will be decided by the majority vote of the Foundation membership present at the Annual Corporate Meeting. In essence, the Members of the Foundation will, on an annual basis, select and direct a significant grant amount to a worthy nonprofit organization. The focus of the 2024 IMPACT FUND grant will be in the area of promoting mental health education. While all of our communities face mental health challenges, Black communities often deal with more discrimination, and can receive compromised care. The lack of education and resources are not available in the Black community when it comes to mental health. The 2024 IMPACT FUND grant will be awarded to the nominated nonprofit organization that best exemplifies, in the eyes of the Foundation membership, “Breaking the Stigma of Mental Health in the Black Community”. We must talk and share information on social media openly about mental health. The more we learn and educate ourselves and other about mental health we can have a positive impact on individuals getting the treatment, care, and support they need. The amount of the 2024 IMPACT FUND grant will be $25,000 and we anticipate the award will be presented in August 2024. The 2024 IMPACT FUND grant application period will begin October 1, 2023 and ends midnight December 31, 2023. All nominations must be submitted by a Foundation Member. The IMPACT FUND Committee (IFC) will review and vet all completed applications and identify a slate of the top four (4) applications for the Foundation membership to ultimately vote on in order to select the one recipient for the grant. It is the goal of the IMPACT FUND Committee to have the top four (4) applications available for the Foundation membership to review by March 31, 2024. Be advised that each of the four (4) finalist will be required to submit a two-minute organizational narrative video. The vote by the Foundation membership will take place at the June 2024 Annual Meeting of the Omega Life Membership Foundation, Inc. OLMF Impact Fund Grant Request Period for each year begins
October 1, 2023 and ends midnight December 31, 2023

Omega Life Membership Foundation, Inc. (Foundation) is accepting IMPACT FUND Applications
from 501c3 Organizations or International equivalents for “Breaking the Stigma of Mental
Health in the Black Community”. While all of our communities face mental health
challenges, Black communities often deal with more discrimination, and can receive
compromised care. The lack of education and resources are not available in the Black
community when it comes to mental health. We must talk and share information on social
media openly about mental health. The more we learn and educate ourselves and other about
mental health we can have a positive impact on individuals getting the treatment, care, and
support they need.


All IMPACT FUND Grant requests directed to OLMF must be submitted on the attached OLMF
IMPACT FUND Grant Request Form for consideration. The request must coincide with our
mission. The application must be completed entirely addressing every area of the form.
The required Grant Request Package must include the following information:


1. IMPACT FUND Grant Nomination Form
2. A copy of the organization’s IRS 501(c)(3) Determination Letter including
the Tax Identification Number or International Equivalent as a Nonprofit
3. Executive Summary of Organization’s Mission and Vision
4. Purpose of the Grant / Grant Narrative
5. Measurable Anticipated Outcomes / Metrics
6. Budget Narrative / Financial Justification
7. Organizational Information
8. Required Attachments
9. Signature Page
